• Select a loan period for each item listed on the form:
  • 2 Hour - in-library use only; must be returned within two hours of check-out time.
  • 3 Hour - can be checked out for home use for three hours.
  • 4 Hour- in-library use only; must be returned within four hours of check-out time.
  • 24 Hour - can be checked out for home use; due 24 hours from the time they are checked out.
  • 2 Day - can be checked out for home use for 2 days.
  • 3 Day - can be checked out for home use for 3 days.
  • 7 Day - can be checked out for home use for 7 days.
  • Term Loan - can be checked out for home use for entire term.

• Submit online form below or type form if you prefer (Word doc) and e-mail to library-reserves@umd.umich.edu.
• As of January 1, 2009, handwritten forms will not be accepted.
